Once again the Seasonal Flu Vaccination Clinics commence. This year they are being held on
Saturday 26th September from 8.30 - 11.30 am
and
Saturday 3rd October from 8.30 - 11.30 am
The Flu vaccine is for patients over the age of 65; a Carer(Carers Form); or suffer from a chronic disease like diabetes, heart disease or breathing difficulties. If you suffer with a chronic disease which meets the criteria you should receive this week an invitation letter asking you to contact the Health Centre to make an appointment. Please bring this letter with you when you come to make your appointment or have your flu vaccination.
Appointments can be made over the phone, in the Health Centre or Online, using Emis Access. (Emis is the name of the software we use to record patient details). If you do not have a pin number to make an appointment online, simply complete this online request form or call in at the Health Centre and ask. Please note the appointments are in General Flu Clinics and not with one particular clinician.
Consent. We also have to obtain consent from patients before administering the Vaccine. Please download this form, Flu Consent Form or pick up from reception, complete and bring with you when you have your flu vaccination appointment.
Carers. If you look after someone who could not manage without you then you are a Carer and need to complete the Carers Form. Please hand it in when you next come for an appointment or post to us.
Loose fitting clothing. It would also help speed up the process if you could expose your arm prior to going in for your vaccination.
Transport. If you live in the vicinity of the Health Centre, it would be appreciated if you could walk to your appointment, if you are able. If you come by car, please park courteously so as not to inconvenience our immediate neighbours. Thank you. |
