Patients living within the practice area (please see practice boundary below) are welcome to register and our reception staff can confirm your eligibility from your postcode. In this respect it would be helpful to bring along proof of address such as a recent utility bill.
You will also need to complete a registration form (GMS1) and a Health Questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. These forms are available from reception. You can fill out the GMS1 form online.

If you currently have medication on a repeat medication scheme you will need to arrange an appointment with a GP to have your repeat medication set up.
